This standard Government form, prescribed by OMB Circular A-102, is used to apply for assistance under this program. Applicants use this form to outline scope, costs and expenses for the project. (0572-0096) There are two attachments to the SF-424. One is a Site Worksheet (required) and the other is a Survey on Ensuring Equal Opportunity for Applicants (optional). The application must be signed by an authorized representative of the organization who has the authority to commit the organization’s resources to carry out the project for the term of the grant.
