Description of Agency Usage: FDIC is subscribing to this common form that will replace the current Standard Form: SF 85, SF 85P, SF 85P-S, and SF 86 which are used for the background investigation process. Until the PVQ is made available for government-wide use via the eApplication ( eAPP replaced eQIP) and it is officially implemented, all agencies will continue to use the Standard Forms. However, in the interim, we are expected and instructed to go ahead and request OMB’s approval to use the PVQ .
