The Personnel Vetting Questionnaire will be used by the Department of Energy and the National Nuclear Security Administration to conduct personnel vetting investigations to determine an individual’s eligibility for continued access to classified matter, special nuclear material, or eligibility to hold a sensitive position. This form will also be used in the same manner to determine an individual’s initial and ongoing suitability, fitness or eligibility for physical and logical access to federally controlled facilities or information systems for Federal or contract employment.
