The Weatherization Assistance Program uses SF-424 Application for Federal Assistance and SF-424A Budget Information as part of the application for funding each year. The SF-424 and SF-424A are built into the Performance and Accountability for Grants in Energy (PAGE) database used for the application and reporting required of Weatherization Assistance Program Grantees. The SF-424 allows Grantees to provide legal and business level information to DOE. Annually, these report forms are submitted for 57 Grantees.
