Title: ●Patient Protection and Affordable Care Act; Health Insurance Market: Rate Review (CMS-9972-F) 
Abstract: This final rule implements the Affordable Care Act's policies related to fair health insurance premiums, guaranteed availability, guaranteed renewability, risk pools, and catastrophic plans. The rule clarifies the approach used to enforce the applicable requirements of the Affordable Care Act with respect to health insurance issuers and group health plans that are non-federal governmental plans. The rule also revises the timing of the submission of requests for State-specific thresholds; revises the standards for health insurance issuers and States regarding reporting, utilization, and collection of data, and amends the requirements for a State to have an Effective Rate Review Program.
