FORM 972 IS COMPLETED BY THE SHAREHOLDER AND FILED AS PART OF THE CORPORATION'S ANNUAL INCOME TAX RETURN. FORM 972 ACCOMPANIES FORM 973 WHICH IS EXECUTED BY THE CORPORATION. THE INFORMATION IS USED TO HELP VERTIFY THAT SHAREHOLERS HAVE CONSENTED TO TREAT CERTAIN AMOUNTS AS DIVIDENDS AND TO REPORT SUCH AMOUNTS IN GROSS INCOME.
