Abstract:
THIS FORM IS USED TO CONFIRM INFORMATION SUPPLIED BY NEW LICENSEES REGARDING THEIR STOCKHOLDERS AND TO DETERMINE THESE STOCKHOLDERS' SOURCES OF FUNDS USED TO ACQUIRE THEIR STOCK. THIS INFORMATION IS NEEDED TO DETERMINE COMPLIANCE WITH THE SMALL BUSINESS INVESTMENT ACT 1958, AS AMENDED, AND THE REGULATIONS THEREUNDER.
