The information collected is required from all laboratories applying for NVLAP accrediation. Applicants provide the minimum information required by published accrediation criteria to achieve effective on-site assessments and informed evaluations leading to sound accrediation decisions.

The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
