Abstract:
This package provides instructions and forms necessary for States to report the number of children receiving early intervention services under Part C of Individuals with Disabilities Education Act (IDEA), the settings in which these children are provided services, and the reasons by which these children exit Part C of IDEA. The form satisfies reporting requirements and is used by OSEP to monitor State agencies and for Congressional reporting.

Short Statement:
In order to make the information reported on the Notice of Office of Management and Budget Action (NOA) accurate, we have adjusted the number of respondents from 168, as previously reported, to 56. OSEP collects the data in this information collection package from 56 states and entities (i.e., 50 States, District of Columbia, Puerto Rico, Virgin Islands, American Samoa, Northern Marianas, and Guam). This adjustment does not represent a change in data elements or response burden; this adjustment is only to ensure an accurate record of respondents.
