Short Statement: As noted in the previous submission, NIGMS is requesting a revision to remove completed surveys and continue the final year of data collection for the DPC evaluation. While the consortium-wide evaluation plan is highly similar to the previous submission, some adjustments have been made to reflect the updated burden estimates for the revised request. Only a subset of the data collection instruments included in the previous submission require continued data collection; therefore, instruments completed as planned during the preceding clearance period have been removed, and the proposed expiration date for the remaining surveys has been extended by 13 months.
